  6. Make sure Off is chosen in the pop-up menu next to the website.
  7. On the left, click Pop-up Windows.
  8. Make sure Allow is chosen in the pop-up menu next to the website.

What you'll do?
  • Coordinate & Oversee Patient Care Plans – Ensure quality, efficiency, and compliance with healthcare regulations
  • Be the Key Connection – Act as the liaison between patients, providers, and support staff
  • Monitor & Adjust Care Plans – Track patient progress and ensure their needs are met
  • Keep Records in Check – Maintain accurate documentation and compliance with healthcare standards
  • Manage Communication – Answer calls, direct inquiries, and follow up on important medical documentation
  • Work with Scheduling Teams – Ensure patient visits and care plans align smoothly
Qualifications:
  • Active LPN or RN license required
  • Extensive experience in clinical coordination, case management, or a related role
  • Comprehensive knowledge of healthcare regulations and patient care best practices
  • Exceptional organizational, communication, and problem-solving skills
  • Proven ability to excel in a fast-paced environment while managing multiple priorities
