The Data Management Coordinator processes the paperwork and information needed to enter or update information associated with a new or existing patient into the different engines that ACH uses.
Through various tasks in Patient Connect, the Data Management Coordinator assists with timely updates of patient information file.

Key Areas Of Accountability
1. Skilled in Basic Data Entry and Indexing.
2. Review, classify and upload all faxed documents received from Warehouse.
3. Validate and enter all new patient information and existing patient updates.
4. Focuses in processing Priority, Critical, Exception & Adhoc tasks.
5. Assist with any special projects/adhoc tasks as directed by supervisor/manager.
6. Reviews Patient Life Cycle in Isynergy from the time we receive a request for a meter to
their treatment ending.
7. Submit Workflow Forms to process needed changes to Patients’ Files (Demographics,
Verbal Authorizations from MD Offices, etc. for other departments to process.
8. Resolves or reports problems encountered immediately, including unreadable data,
unacceptable data, and any other problems to their management team.
9. Follow all regulatory policies and procedures, privacy and security standards in
accordance with Government agencies to include HIPAA requirements.
10. Understand and adhere to business processes and procedures.
11. Maintain a culture of accountability in area of responsibility.
12. Requires minimal Supervision by the Supervisor/Manager to be successful.
