HHA Coordinator - Registered Nurses or Nursing Grads

Responsibilities:

  • Maintain a patient tracker for each assigned Home Health Aide.
  • Check-in with each HHA weekly to address any questions or concerns.
  • Assist with updating schedules in Kinnser and CareStitch.
  • Follow up with HHAs after completing their first visit.
  • Alert the HHAs when theres a new Case Manager.
  • Keep track of patients that are placed on hold due readmission or per PT/family request.
  • Confirm if HHA will continue after ROC visit, and if Case Manager provided an updated Aide Care Plan.
  • Monitor authorizations for HHA visits.
  • Monitor RECRT period.
  • Monitor missed visits.
  • Submit a Cancelation of Discipline order for the Case Manager.
  • Monitor the HHAs if they are following the F&D.
  • QA - chart review
  • Other tasks that may be assigned to you by the manager.


Qualifications:

  • Must be a Registered Nurse or a graduate of Nursing.
  • Clear speaking voice.
  • Good English communication skills - both written and verbal.
  • Ability to exercise initiative and independent clinical judgment.
  • Ability to work with individuals, to enlist the cooperation of many people to perform/achieve a common goal.
  • Strong customer service skills.
  • Strong computer skills - accurate typing & spelling is essential to this role.
  • Energetic, fun, and confident personality.
  • Looking for a long-term position.
