Job Description: Home Care Agency RN/Case Manager (Virtual)
Position Overview:
Nu Grace Family Care is seeking an experienced and detail-oriented Virtual RN/Case Manager to join our team. The ideal candidate will have prior experience in home care or home health, strong computer skills, and excellent customer service and organizational abilities. This remote position is critical in ensuring clients receive the highest quality of care while maintaining compliance with agency standards.
Compensation:
• Hourly Pay: $3.50–$4.00
• Paid weekly on Fridays
Key Responsibilities:
• • Develop, review, and update individualized care plans tailored to client needs.
• • Collaborate with caregivers and monitor their performance to ensure adherence to care plans.
• • Conduct remote assessments and identify changes in client conditions, making necessary updates to care plans.
• • Act as the primary liaison between clients, caregivers, and family members.
• • Provide timely and professional responses to inquiries and resolve concerns.
• • Maintain detailed, accurate, and timely records of client care and communications.
• • Prepare reports as required for agency compliance and operational needs.
• • Use software and online tools for scheduling, care plan management, and communication.
• • Ensure secure handling of client data in compliance with HIPAA regulations.
• • Work closely with the agency’s scheduling team and staff to ensure seamless client care.
• • Coordinate with healthcare professionals and external agencies when necessary.
• • Ensure all care activities comply with agency policies and applicable state and federal regulations.
• • Monitor caregiver compliance with care plans, training, and agency standards.

This is not inclusive of all duties. Training will be provided as needed, but you must be a fast learner.

Qualifications:
• • Active RN license
• • Required: Minimum of 1 year of experience in home care or home health.
• • Strong computer skills, including proficiency with scheduling software, Microsoft Office Suite, and video conferencing tools.
• • Excellent organizational and multitasking abilities.
• • Strong verbal and written communication skills.
• • Must be able to work independently.
• • Reliable internet connection and a quiet home office setup.
Preferred Qualifications:
• • Bilingual skills are a plus but not required.
• • Familiarity with non-medical home care services.
Schedule:
• Flexible hours based on agency needs (PST Do not apply if you cannot work PST hours).
