Job Summary & Responsibilities

An Intake coordinator plays a vital role in ensuring a seamless experience for clients seeking therapy services. Intake is responsible for processing incoming patient referrals in the form of calls, email s, and faxes by gathering the appropriate preliminary information such as patient demographics, insurance details, and home health related diagnoses.

An Intake Coordinator serves at the primary point of contact for our clients, responding to inquiries, provides information about our services, and acts as a liaison between the Assigning Department and our partnering agencies giving ongoing updates regarding a referral status.

Overall, an Intake coordinator facilitates a patient’s successful transition into our care or identifies our company’s inability to help a patient due to coverage, scheduling, or other types of incompatibilities.

Qualifications and Skills

• Have excellent communication, and customer service skills- verbal and written
• Have a typing speed of 50WPM or faster
• Be detail oriented and organized
• Have familiarity with various home-health related diagnoses
• Work well on team
• Be resourceful and solution-oriented
• Be fun and outgoing!
